These specialized health navigators are committed to improving health outcomes through a proven process. They will identify patient health goals and barriers, create an achievable care plan of action and boost self-management confidence. Primary services provided:
Remote Therapeutic Monitoring (RTM): We use technology to keep track of your medication usage and follow up based on adherence to what you've been prescribed, so we can adjust your care as needed.
