How It Works

Through our services, a health navigators will be in direct contact with each enrolled patient and your clinic or community pharmacy. These specialized health navigators are committed to improving health outcomes through a proven process. They will identify patient health goals and barriers, create an achievable care plan of action and boost self-management confidence. Primary services provided:

Remote Therapeutic Monitoring (RTM): We use technology to keep track of your medication usage and follow up based on adherence to what you've been prescribed, so we can adjust your care as needed.

Remote Patient Monitoring (RPM):
We utilize cutting-edge technology to monitor patient’s health in real time. From tracking vital signs to detecting early warning signals, our RPM services are tailored to improve patient well-being and reduce hospital readmissions.
